In I’d Leave My Happy Home For You, it is revealed that Alaric and Jo would have had twins if Jo hadn’t been killed by Kai. However, the Gemini coven saved their children by putting them in Caroline Forbes; who gave birth to their twin daughters; Josette and Lizzie Saltzman in the future.
As it turns out, the TVD writers came up with an ingenious way to write while pregnant, even though vampires can’t get pregnant. After Kai butchered his sister Jo on the altar, having just said “yes” to Alaric, the twins she was carrying were spell saved and implanted in someone else.
Alaric and Caroline name their new daughters Lizzie Saltzman and Josie Saltzman, after people who loved them who died (Caroline’s late mother and Alaric’s former fiancé). Later Alaric and Caroline take the babies and move to a new home far away from Mystic Falls.
Jo was killed by her own twin brother, Kai, while Lizzie and Josie were still in her womb. The twins only survived because they were magically transferred into Caroline’s womb. So the twins have two mothers, but – ironically – the undead Jo will be the first to appear in Legacies.
In their final moments, the members of the Gemini Coven performed one final spell. Kai thought it was an attempt to banish him, but they actually saved the babies by putting them in a surrogate. While Caroline was unable to father a child, she was able to carry one – in this case two – once in the womb.

Long story short, the twins discovered that when they turn 22, they must participate in the fusion, which is essentially a fight to the death for witches. (The stronger witch absorbs the powers of the others.)

The Alaric/Jo wedding was truly a bloodbath as the finale officially confirmed Kai (Chris Wood) killed Jo (Jodi Lyn O’Keefe) — and possibly Alaric’s (Matt Davis) will to live – and her unborn babies.

Thanks to some very powerful magic, Caroline now carries the twins of Jo and Alaric, aka the only two remaining witches in the recently disbanded Gemini Coven, so she’s really just a vampire surrogate.p>
